Award Nomination Forms

To complete, please select an award form from the options below. From there you’ll have the option to download and save the PDF to your computer. After saving you should open the document on your computer using Adobe Acrobat (make sure you click the “Highlight Existing Fields” tab in the upper right-hand corner) and fill in your responses into the document. Once completed, save and e-mail the document to Heidi Glanzman at glanzman.4@osu.edu.

CARL of the Year Award (Societies Only)

The 2017 Outstanding Alumni Society CARL Award for service to the alumni society is presented to that staff member who serves the alumni society as the Campus Alumni Relations Liaison. This is a chance to recognize that staff member who gives of his/herself to the greater well being of the alumni society.

Please consider the following areas when submitting your nomination:

  • Actions above and beyond their job description
  • Examples of their dedication to the alumni society
  • Service to the alumni society for a minimum of 3 year

Innovative Program of the Year Award

The Most Innovative Program of the Year award celebrates new and unique programming initiatives planned and executed by Scarlet level Ohio State University alumni groups. The nominated program should:

  • Advance the mission and values of The Ohio State University Alumni Association, Inc.
  • Advance the university as a whole, while also supporting the goals of the alumni group.
  • Program must be considered a category I event.
  • Event must be solely planned and executed by the alumni group.
  • Each group may nominate only one program which occurred in fiscal year 16.

Pay it Forward Award for Volunteerism

Ohio State University alumni groups are encouraged to identify and nominate one worthy individual who, through their service and achievements as a club or society leader, goes above and beyond to support and advance the goals and objectives outlined within the strategic plan of The Ohio State University Alumni Association, Inc. The nominee must be a current or past club/society leader and a sustaining or life member of the OSUAA. Each group may nominate one individual per year. Up to five club and five society leaders may be selected each year for this award. Please consider the following areas when submitting your nomination:

  • Leadership and service to OSU and the alumni group
  • Initiatives created under this individual’s leadership
  • Actions taken above and beyond their duties as a member or officer
  • Significant contributions made to the alumni group
  • Length of time serving on the board or committee of the alumni group

Program of the Year Award

The 2016 Program of the Year award celebrates a programming initiative that is superior to all others. Up to three club programs and up to three society programs will be chosen for this award. Each group may nominate more than one program, but can only be recognized once. This program should:

  • Advance the mission and values of The Ohio State University Alumni Association, Inc.
  • Advance the university as a whole.
  • Support the goals of the alumni group.
  • This program must be considered a category I event an occurred in fiscal year 16.
  • Event solely planned and executed by the alumni group.

Alumni Group of the Year Award

All Scarlet level Ohio State alumni groups are encouraged to apply for either the Club of the Year or Society of the Year Award. Clubs and societies of Scarlet level are automatically recognized as outstanding groups; from there, ONE club and One Society will be selected and recognized as Club of the Year and Society of the Year. Groups that receive this award must exemplify the mission and values of The Ohio State University Alumni Association, Inc. and university as a whole, with volunteer efforts that exceed expectations and excel in all areas. Additionally, to be recognized, group leadership must have submitted the following items to the appropriate staff member prior to nomination:

  • Board member and general membership rosters
  • All events and their descriptions (photos recommended)
  • Name and venue of all social media pages (please provide URL)
  • Constitution and charter (must be up-to-date and on file)
  • Contributed to student recruitment, retention, and/or scholarships
